Interoperability of health record is also a major concern in the guidelines provided by ministry of Health and Family welfare. In India there is no any effective model adopted by various medical practitioners and healthcare service centre. Most of the countries are having their own effective electronic health record system. Patient record is maintained on central servers in prescribed format. Patients visit the doctor or at a medical facility Centre for some health related problems.
