Der vorliegende Teilband des Handbuchs der inneren Medizin bringt aus der Thematik der Blutgerinnungsstörungen und hämorrhagischen Diathesen den Abschnitt der plasmatischen Hämostasedefekte, der Koagulopathien, zur Dar- stellung. Innerhalb der letzten beiden Dekaden hat die Hämostaseforschung eine Unzahl von entscheidenden Impulsen erfahren, die eine Fülle von Erkennt- nissen über die ätiologischen Determinanten und pathogenetischen Mechanis- men der Blutgerinnungsstörungen erbracht haben. Dank der enormen metho- dischen Fortschritte in der biochemischen Grundlagenforschung ist es gelungen, für die meisten angeborenen Koagulopathien den molekularen Defekt der plas- matischen Komponente zu identifizieren. Gerade die detaillierte Analyse ver- schiedener Varianten einzelner Gerinnungsfaktoren, so z. B. beim v. Willebrand- Jürgens-Syndrom oder bei der Dysfibrinogenämie, hat einen tiefen Einblick in molekular-genetische Zusammenhänge erlaubt und subtile Kenntnisse über den Metabolismus des Hämostasesystems und die Dynamik seiner biologischen Regulation hervorgebracht. Die Steuerung der Aktivierungskinetik im Gerin- nungs-und Fibrinolysesystem und ihre Kontrolle durch den vielschichtigen Ein- fluß verschiedener Inhibitoren hat belegt, daß enge Wechselbeziehungen zu an- deren protektiven Enzymsystemen des Organismus, wie z. B. dem Komplement-, dem Kallikrein-Kinin-System und den Phagozytosemechanismen, bestehen. Die grundlegenden Erkenntnisse über die Pathogenese und Pathophysiologie der gestörten Hämostasefunktion haben nachhaltig die Entwicklung effektiver therapeutischer Maßnahmen in Gang gesetzt. Die Möglichkeit der gezielten Rekompensation des Hämostasepotentials durch Substitution mit gereinigten Gerinnungsfaktoren oder die Unterbrechung einer Umsatzsteigerung mittels geeigneten Inhibitoren garantiert heute die Beherrschung nicht nur akuter, le- bensbedrohlicher Blutungskomplikationen, sondern auch die segensreiche Dauertherapie bei angeborenen Blutungsübel, wie z. B. der Hämophilie.
