Dans ce livre, Freud propose l'une des premières descriptions de la méthode propre à l'interprétation analytique. De fait, il décrit la règle de l'association libre qui restera, encore aujourd'hui, l'un des piliers essentiels de la pratique psychanalytique. Freud prend appui pour indiquer ce qu'est la technique de l'interprétation plusieurs de ses propres rêves: Le rêve de l'injection faite à Irma, est le plus essentiel, celui qui lui sert de référence, mais il y en a d'autres encore. Par exemple Le rêve du Comte de Thun, qui rend compte de l'érotisme urétral de Freud ainsi d'ailleurs que le rêve dit du W.C de campagne. Si Freud nous indique qu'il a avant tout choisi ses propres rêves, c'est parce que choisir d'autres rêves de névrosés aurait nécessité d'autres détours théoriques pour rendre compte des liens du rêve avec les symptômes. Mais il n'empêche qu'il en rapporte quelques-uns, celui par exemple d'une jeune femme agoraphobique, qui dans son rêve, s'était fabriqué un chapeau avec les organes génitaux de son mari, et pour se protéger de son désir de se laisser séduire par de beaux militaires. Le rêve ne révèle pas l'avenir: il ne s'agit pas d'un présage sur lequel le rêveur pourrait s'appuyer. Au contraire, le rêve révèle, justement, le passé, celui du rêveur. Le désir inconscient correspond à la sexualité infantile, énigme dont le névrosé ne comprend plus les éléments, qu'il refuse de concevoir, ces représentations inconscientes étant inconciliables avec les exigences de la conscience.
