Esta formidable compilación de cien disyuntivas, enfoques e interpretaciones de sucesos, ha sido escrita por el eminente dermatólogo, académico, tanto de la Medicina como de las Ciencias Físicas, Matemáticas y Naturales, eficiente diplomático accidental y valioso hombre de inmensas inquietudes, Francisco Kerdel Vegas y está complementado por dos análisis de trípticos de paradojas, realizados por los Doctores Augusto León C y J. M. Avilán Rovira.

Allí encontramos todo un catálogo, con sus estupendas definiciones y eficiente análisis, de una diversidad de situaciones en las que con toda seguridad, nos hemos visto envueltos en diferentes etapas de nuestra vida.

La medicina, que no es solo una ciencia, durante su ejercicio tanto por parte del profesional actuante como por la del paciente y sus familiares, se ve envuelta en una diversidad de encrucijadas que tienen que ser analizadas tanto desde el punto de vista científico como desde sus innumerables implicaciones económicas, sociales, éticas, filosóficas y morales.

Con esto queremos decir e interpretar que el formidable trabajo que realizó Kerdel al establecer cien paradojas en la medicina moderna, se nos presenta como todo un tratado de humanidad y de comportamiento ante la sociedad. De respeto a la dignidad del hombre. De evaluación entre lo posible y lo deseable. De análisis del conflicto entre lo rentable y lo ético.

Especial interés nos surge al leer los análisis de las situaciones que se presentan cuando se acerca la muerte. La gran disyuntiva que aparece diariamente en los casos de enfermos terminales donde el conjunto de profesionales que lo atienden y los familiares más allegados al enfermo deben escoger el camino a seguir, ante la posibilidad de aplicar lo que hoy recibe el pomposo nombre de medicina heróica y que no es más que la prolongación de la vida por medios absolutamente artificiales.

Encontramos también un análisis de lo que se denomina la calidad de la vida, parámetro que pensamos, debe de estar muy presente en los argumentos que se expongan en las horas finales de un enfermo.

Como colofón de estas situaciones, Kerdel analiza la confrontación entre la vida vegetativa y la eutanasia, eventos que lo llevan a analizar lo que en los últimos años se ha denominado el "derecho a una muerte digna".

También nos llamó a especial atención los eventos que también se suceden con frecuencia diaria y que son los de prescripción de análisis innecesarios y costosos que van mucho más allá de lo requerido.

Paralelo a esto, se analiza el desideratum entre la debilidad o carencia económica del enfermo y sus expectativas de una atención médica de calidad.

También leemos el conflicto entre donación o comercio de órganos; las diferencias entre la atención que brinda el médico familiar y las ofrecidas por el médico especializado. Un tema que aunque está sobre el tapete tiene tantas implicaciones que se trata con la mayor prudencia, es el de los embarazos asistidos, el de las manipulaciones genéticas y las situaciones de aborto.

Tampoco escapa al análisis del Dr. Kerdel-Vegas, el adentrarse a los distintos enfoques y conflictos que se pueden presentar y de hecho ocurren con mucha frecuencia, entre las empresas aseguradoras y los profesionales de la medicina.

En fin, nos encontramos ante todo un tratado de humanidad y de comportamiento ciudadano que mucha falta le hace a nuestro mundo actual.

About the Author: Dr. Francisco Kerdel-Vegas is an internationally known dermatologist whose work and research has been awarded numerous national and international awards. He has authored five textbooks and over 150 articles in the fields of tropical and dermatological diseases. Having done his early schooling in Caracas, he completed his medical studies at the Central University of Venezuela in 1951. Following this, he became a clinical fellow and graduate student in dermatology at the Massachusetts General Hospital, at Harvard Medical School in Boston, and he continued his dermatologic training at the New York Skin and Cancer Unit and at the New York University Postgraduate Medical School (1952-1954). He was awarded a master's of science in dermatology and syphilology from New York University. On his return to Caracas in 1954, he became assistant professor of dermatology at his old alma mater, the Central University of Venezuela. He was appointed professor in 1961, and was made doctor of medical sciences at this university in 1966. He then was a visiting scientist at the department of Experimental Pathology at the Institute of Animal Physiology (Agricultural Research Council, Cambridge, England), and became a member of Trinity College, University of Cambridge. In 1968, he became the first pro-vice-chancellor of the newly created Universidad Simon Bolivar, and a member of the National Research Council of Venezuela. Life Fellow of the National Academy of Medicine of Venezuela since 1967. In 1970, he became a member of the executive committee of the Metropolitan University, Caracas, and in the following year, 1971, he was appointed Life Fellow of the Academy of Sciences of Venezuela. Since 1972, he has been Fellow of the American College of Physicians, in Philadelphia. He was awarded the honorary degree of D.Sc from the California College of Pediatric Medicine, in 1975. Honorary D.Sc. from the University of Cramfield, U.K. (1998). He has served his country of birth as Ambassador to the United Kingdom (1987-1992), France (1995-1999) and UNESCO (1994-1999).
