Die Produktion von Stickstoffmonoxid (NO) in vaskulären Endothelzellen ist inzwischen zu einem integrativen Bestandteil der vaskulären Biologie gewor- den. Einige der wichtigsten Effekte, die NO in der Blutgefäßwand auslöst, können als potenziell vasoprotektiv angesehen werden, denn sie dienen der Erhaltung wichtiger physiologischer Funktionen. Hierzu zählen im Wesent- lichen vasodilatator ische, antiaggregatorische, antiadhäsive, antiproliferative und antioxidative Effekte. Während der letzten 20 Jahre wurde immer deut- licher, dass viele kardiovaskuläre Erkrankungen mit einer deutlichen Ein- schränkung der Funktion des vaskulären Endothels verbunden sind, die sich u. a. als verminderte Produktion und Bioverfügbarkeit von NO äußert. Einer der wichtigsten Gründe hierfür ist vermutlich die bei solchen Erkrankungen nachweisbare Steigerung der vaskulären Produktion freier Radikale, ins- besondere Superoxidanionen. Diese Radikale können auf unterschiedliche Weise mit dem NO/cGMP-Signaltransduktionsweg interferieren und dessen Effizienz stark beeinträchtigen. Ein solcher Pathomechanismus wurde für so verschiedene Erkrankungen wie koronare Herzkrankheit, Herzinsuffizienz, Hypertonie und Diabetes mellitus beschrieben. Die Intention dieses Über- sichtsbandes besteht darin, die zellulären und molekularen Mechanismen zu beleuchten, über welche NO potenziell vasoprotektive Effekte entfaltet. Darüber hinaus wird die Rolle der endogenen NO-Bildung für die Entwick- lung kardiovaskulärer Erkrankungen diskutiert. Schließlich werden pharma- kologische Möglichkeiten zur Beeinflussung des NO/cGMP-Signaltransduk- tionsweges in der Absicht abgewogen, ihren Stellenwert zur Verzögerung der Pathogenese kardiovaskulärer Erkrankungen näher einzugrenzen. In diesem Zusammenhang hat sich das organische Nitrat PETN als inte- ressanter Arzneistoff mit möglicherweise protektiver Wirkung auf die durch oxidativen Stress ausgelösten- Gefäßschäden erwiesen.
