Krankheiten, welche heute keine bedeutende Prävalenz mehr besitzen, aber durch ein anderes Phänomen ersetzt wurden. Der Rückenschmerz, besonders in seiner chronischen Ausformung, ist zu einem ausufernden, sozioökonomischen Problem der hoch entwickelten Nationen geworden (vgl. Göbel, 2001, S. 93). Grönemeyer (2006, S. 15) zeigt einige wichtige Fakten auf, welche die Brisanz der Situation verdeutlichen: - 71,5 Millionen Arbeitstage fallen wegen Rückenschmerzen jährlich in Deutschland aus - 35 Prozent der jüngeren Bevölkerung haben vorgefallene oder degenerierte Bandscheiben - In Deutschland werden jährlich 60.000 Bandscheiben operiert Rückenleiden bedeuten folglich nicht nur für das Individuum eine Bedrohung, sondern fordern explizit das Gesundheitssystem und seine Partner heraus. Sowohl Ärzte, Therapeuten und medizinisches Personal, als auch die Kostenträger stehen den wachsenden Inzidenzraten bisher nahezu machtlos gegenüber. Treffend äußert sich Waddell (zitiert nach Grönemeyer, 2006, S. 19), dass "[...] Rückenschmerzen die medizinische Niederlage des 20. Jahrhunderts seien". Eine Ursache jener unbefriedigenden Lage kann in einem vorherrschenden, kausalen Denkmodell gesehen werden. Der Glaube eines "[...] linearen Zusammenhanges zwischen körperlicher Schädigung und empfundenem Schmerz bzw. einhergehender Funktionseinschränkung [...]" (Kröner-Herwig, 1999, S. 68) ist weitestgehend in den Köpfen aller Beteiligten verankert.
